All 2008 1098 Tax Forms will be mailed on or before January 22, 2009. If you do not receive your 2008 1098 tax form, please do one of the following:

• Login to our secure site and print your 2008 1098 Tax Form free of charge.
• For a fee of $15 you can order a replacement 1098 form.
• Call 1-800-970-7250 to request a duplicate from one of our representatives.
• Call our toll free number and using our automated system select your language
  preference then choose option 36 to obtain the information.
